How   to   register


To officially register for the team after the application process, please follow these steps:​
  1. Complete and submit our Registration Form.
  2. Read the Student Handbook. Print, sign, and turn in the student contract on the last page.
  3. Print, fill out, and email the DJUSD Medication Assistance Authorization. The first half of this form must be filled out regardless whether or not you have any medications. This form must be signed by a parent or guardian as well.
  4. Print, fill out, and email the DJUSD Voluntary Activity Participation Form. 
  5. Print, fill out, and email the DJUSD Volunteer Personal Automobile Use Form if your parent(s) might consider driving. Driving is highly encouraged, but not required. Do not forget to attach a photocopy of your Driver's license and Insurance Policy Card/Declarations Page.
  6. Complete and submit our Meal Preferences Form.
  7. Complete online STIMS registration, including parent registration, the 2025-2026 FIRST consent and release form, and team application.
​If steps #1-7 have not been completed and turned in by August 29, students will no longer be invited to join the team. Please scan and send all forms to [email protected] OR bring physical copies to New Member Orientation on August 28rd. Thank you!​
